Sponsorship and Gift Aid

Request forms by post

You can request any of the Relief Chest forms or documents listed on this page by emailing reliefchest@mcf.org.uk and providing the information as laid out in the email.  Alternatively, you can make your request by telephoning 020 3146 3333.

These will be delivered to you free of charge.

Please note: Vouchers for making payments from a Relief Chest to a charity can only be requested by the Relief Chest representative.
